Applicant declares that the information provided in this application and copies of attachments are true and correct.
Applicant hereby agrees that in the event this application is approved, subscription to the package applied for has a TWO-YEAR lock-in period. A pre-termination fee of ₱2000.00 for SD bundle and ₱4000.00 for HD bundle shall be applied should disconnection be made within the lock-in period, in consonance with Section 2.6 of Memorandum Circular No. 05-06-2007 Consumer Protection Guidelines of the National Telecommunications Commission.
No installation of the Subscriber Premises Equipment (CPE) shall be made without the corresponding payment for said box together with the installation / activation fees and the updated initial subscription fee, unless waived. Subscriber Premises Equipment (CPE) means the hardware which is required to access the service.
Applicant hereby agrees that this service is subject to the Cybercrime Prevention Act of 2012.
The provisions of this Application shall form part of the subscriber’s Service Agreement once approved. Any failure on the part of Cebu Cable to enforce any of the provisions of this agreement shall not be construed as a waiver.
Applicant hereby accepts and agrees to the terms and conditions of this contract.
Applicant consents to the collection, processing, and storage of their personal data in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) and its implementing rules. The Applicant acknowledges that their personal data will be used for the purposes specified in this Agreement and understands their rights regarding access, correction, and deletion of their data.
